The gay lisp/accent is theorized by some to be due to gays hanging around women more than men and just picking up the speech traits from them.

If the fag is dressing as ridiculous as in that picture then I would imagine he's hanging around mostly girls and as a result he'd be sounding a great deal like them, leading to a hyper version of the gay lisp.

A lot of gays go through some identity crisis when younger about how it's appropriate to behave as someone that's gay and so it doesn't seem all that surprising to me that they may dress oddly as a result.
