b) humiliation, embarrassment, taboo, feelings of submission, loss of control and secret deviancy
While some will claim that they only enjoy one or the other of these parts (there are strict "nonsexual" "innocent" adultbabies out there, as well as "exclusively S&M side" diaperlovers), the truth is that every single person feels and enjoys both of them, albeit to varying extents.
Both parts contain psychological and physical effects.
First of all psychologically a) results from the feeling/smell/view/knowledge of being in a diaper triggers dormant memories of early childhood, which most people associate with the aforementioned feelings of calmness, security and safety. Furthermore the knowledge that one can let go of the control over ones bodily functions most likely automatically triggers memory patterns in the brain, of times when such control was not needed, thus making it discard normal everyday worries faster, and adapting those worry-free thought processes into everyday thinking. Responsibilities lose their mental value, and worrisome things are simply forgotten.
But there is also a more physical side to this: The soft, thick and ideally warm fabric against some of the most sensitive parts of the human body automatically relaxes the entire nervous system, which amplifies the effects of the psychological factors, because the softness is a constant that is interrelated with the triggered memories even when one does specifically think about them all the time.
The b) part is similarly split. The most immediate effect on the wearer is the physical sensation of the diaper. It is not only a soft calming touch, and a reminder of childhood, but, since we've also all reached sexual maturity, also a constant stimulation of the genitals. I do not think i have to explain why having something soft and warm and often squishy and wet rub against your nether regions with every single movement is mostly experienced as sexy.
The most obvious effect for the onlooker however should be the humiliation and the embarrassment, and the submissiveness of the situation. There is one more physical effect here. Diapers do restrict movement, because of improper or impossible closure of legs, a waddling gait due to the enlarged crotch and butt areas, and the constant feeling of having something unusual and unknown between ones legs whenever one tries to close them even a bit. These all reinforce feelings of submission and loss of control for the person wearing the diaper/s.
The last remaining effects are the psychological effects which arise simply out of the fact that diapers are not widely regarded by society as normal clothing items for adults, which makes wearing them taboo, and because of the fear of other people's reactions also very embarrassing. If the situation is made more extreme and the fear of being exposed is made a reality, the embarrassment is replaced with humiliation, because of everyone knowing about the taboo but fun little secret.

The really interesting part however is the fact that the explanations given above have very little to do with personal preference. Granted, the individual weighing of the elements may vary based on traumas and life experience in general, and thus someone may not find one specific combination of sensations as pleasurable as others, and might like something else more. However the basic mechanisms responsible for liking these things are invariably present in all humans, and that a non negligible degree.
This means that based on strict rationality, pretty much everyone would experience the described situation as pleasurable in most of the parts.
