Can Time Lords choose their appearance whenever they regenerate or is it always random?

And if The Doctor's regenerations are random because he's a novice time lord, does he eventually get an understanding of how to regenerate in order to become The Curator?

Most Timelords just seem to regenerate when they're getting old and feel they need a new body. Being on the brink of death probably makes it harder to control.

I'd love to see them explore what happens to a Time Lord who lives for too long. Presumably there's a reason regenerations are usually capped at twelve.
