Sometimes doctors will apply maggots or other flesh eating bugs to rotting tissue in order to combat necrosis.

In this case though, I couldn't be certain:

On one hand, the opening is swolen and the maggots are well-established in the wound, which isn't typical of that sort of treatment. There's also dirt, which implies long-term exposure, meaning that the maggots could have developed naturally.

On the other, there are blood stains that look to be fairly recent. One could argue that they could be the result of a doctor tampering with the wound, but one could also argue that the blood is the result of an open wound.

Like I said, I couldn't be certain, but I think that the maggots have intentionally either been administered or left to eat the dead flesh. The latter being more plausible.
