The First Reich was the Holy Roman Empire, the second Reich was Germany as unified under the Prussians, and the third Reich was Nazi Germany.
The entire Reich (=Empire) thing is based on multiple smaller nation-states that supposedly belong to a larger German Empire. The Third Reich took its name both to establish itself as the legit succesor state of the Second Reich (that is, the German Empire of 1871-1918) and because it also "unified" various European states under one rule, be it by a voluntary Anschluss (such as in the case of Austria) or by less-voluntary occupation/annexation (Denmark, Bohemia, Poland, France, etc.)
The entire Reich idea also nicely played into Hitler's hands as the Germans' disappointment in the weak democracy of the Weimar republic made them long for a non-constituional monarchy against, so Hitler had little difficulty to portray himself as a new Fuhrer (or rather - the Emperor of the Third Empire).

PS: While the Weimar Republic's official name indeed was "Deutsches Reich" (=German Reich/Empire) because it was the legal successor of the Second Reich, the Nazis were not overly fond of Weimar's democratic structures and thus promptly ignored it's official title, declaring Nazi-ruled Germany as the actual "Third Reich".
This counting error could have been rectified by later historians, but for some reason, the entire Reich fad rapidly lost popularity after WW II and things were left as they were.

Third Reich is only a unofficial naming convention because of the administrational changes from Monarchy to Republic to Dictatorship. NSDAP Germany was still a legal continuation of the Second Reich.
Also Reich =/= Empire. It more closely translates to realm actually. The term does however also get applied to an empire which makes it confusing.
