Make a list of things you need to get done but are having a hard time getting up and doing.
Break down each of the tasks in your list into multiple segments. For instance if your task was to write a 10 paragraph essay, you could break it down so on Monday you do the opening paragraph, on tuesday you do the next two paragraphs and etc etc. The purpose of breaking down the tasks is to make them less overwhelming. Once you start doing each of the small segments piece by piece, that feeling of accomplishment will help you finish. Also when a person starts something they are much more likely to finish it.
Punish yourself if you avoid doing your task and reward yourself if you do them. For instance if you have to go gym and you don't end up going, unplug your internet router until you end up going. Or if you have gym and you go, reward yourself by putting on your favourite show or whatever.
When you break down the tasks into smaller segments, set a deadline on each of them. So for segment 1 it would be by monday, segment 2 by tuesday etc. Treat said deadlines as if they are the real deal.
Whatever it is that you end up doing when procrastinating, make it much harder to do. So for instance if you end up going on 4chan or 8chan instead of doing your work, delete their bookmarks and make it as hard as possible to access them. Or if you end up playing video games, unplug your shit and put the wire somewhere where it would take effort to get to.

As a lifelong procrastinator I have to say there is only one way to stop procrastination. Stop procrastinating. If you have something to do, just do it. There is no magic trick, no way to constantly motivate yourself. Just bite the bullet, start doing things, see they aren't that bad, and finish. Keeping a daily planner also helps because it feels good crossing things off.
