Not really the same as OP but here a daily task sheet I made. Think of it as daily achievements. It's a simple plan.

Pic is just an example. I can add other task to the sheet. Instead of marking each task with success or fail, one could also fill the cells with the number of hours used or number of times you do something (e.g. push ups).

I'm open to suggestions.

I keep a very similar record in my organiser for each week, with rows for "Effort", "No Fap", "Get up before 10am", and "Bed before 1am".

My get up / bedtime goals used to be earlier, but I could never achieve them regularly and seeing entire weeks of X's was really getting me down so I shifted them.

I still fail to hit them 75% of the time, but oh well.
