Sounds like the biggest problem is that you're weak-willed.
So let's get to work on that right now. Get down and start doing pushups as soon as you're done reading this post. Keep doing them until you feel like you can't do anymore, then keep pushing. Do them until you've collapsed onto the ground, then keep pushing. Even when your body has stopped moving at all, keep fucking pushing. You are going to teach your body to obey your mind, no matter what it feels like doing. When there is not a modicum of energy left in your arms, get up and start doing squats. Same routine. When you can't squat anymore, prop yourself up on something with your arms and keep going. When you can't do that anymore, fall over and start doing situps. No resting. Keep pushing even if you're not moving. When there's no push left, take a rest until you have the energy to get up off the floor. Get up when you can and go drink some water and eat something with protein in it.
In addition to training action, you will need to train inaction. Go to the shower/shitter/wherever you go to fap and just stand/sit there. Refuse to move a single inch until you are ready to leave. Do not scratch itches, don't play with your hands, don't do anything but breathe.
