I've always liked the idea of a game that would try to encourage the player into holding. Perhaps rewarding their party with more MP or luck (increasing as time goes by), or something equally as silly. I can imagine players almost peeing themselves just to defeat a boss, or to get through a tough cave or forest. The final boss would be great fun. Of course, a player would have to be honest about when they use the toilet.
I've noticed it's not a popular angle for these kinds of games, I've only really seen anything close to it in those holding challenges on milovana.
