What you forget is that with world two, just because equality exists doesn't mean it has to be practiced. Part of being a Libertarian world is the right to discriminate which creates a survival of the fittest where those who get frequent abortions. Without the right to discriminate, you can't call it a "libertarian" world, faggot.
Now with the right to discriminate in place, while you can suck a bag of dicks all day 'erry day, hospitals will deny you service for being a fag while the ones that will accept you still have an incentive to charge double the rates because they're the only one that accepts you. Suddenly your AIDs medication, no longer covered by the government dime, costs a shit ton of money since after 50 years of this Lolbertarian paradise, only homogenous traditional families survived. That means the hospitals are fully aware that only degenerates need the medicine, eliminating competition since it's a niche market, and allowing them to charge out the wazoo since degenerates need it to survive (or at the very least to make life a lot less painful for them). Abortion is easily available, but you have to pay out of pocket for it, meaning cheap sluts can't afford it and get burdened down with their kids, keeping them from taking the D every which way. On the bright side, the costs of common practices like kidney transplants or hip replacements now cost a fraction of the price because individuals have a direct relationship with their doctors/hospitals, and said doctors/hospitals offer personalized insurance. Cat girls are on the horizon, and genetic medicine is already used to treat numerous diseases that you would never imagine since hospitals are competing to create new procedures to help people, hiring Uni students to conduct said research in the process. Speaking of which, since Women's Studies has no real world value, it's been dropped from the Uni programs almost completely outside of one or two niche schools in California or New York. Trade skills are extremely popular, and parents enjoy teaching their kids numerous things since they now have a 20 hour work week (unless they want to work more) due to automation. Hell, in some cases the parents home school their children or at least have a personal relationship with the teacher that exceeds public schooling by numerous standard deviations. Competition between schools ensures that prices stay low, and children receive a wholesome Christian education (including evolution, before some faggot tips his fedora). Women are allowed to work in the workforce, but do so because they want to rather than because they need to.
Since we're talking Libertarian and not Voluntarist, there might even be a universal basic income in place.

As for world one? I enjoy watching them struggle to have the state fix everything for them before realizing that Christ was an anarchist (or at least to a large degree a Libertarian). After all, if there's one lesson that y'all should have gotten from the Bible, it's that man is an imperfect being, so why do you trust his judgment to be perfect when used on a collective?
