It must feel horrible that it's not easy for you to just simply find out. Last year when my friend's cat was really ill, he said the worst thing was having it go to the vet and then not knowing if it would come back. When he found out it had to be put to sleep, he was upset and cried in bed all day, but he was relieved because he at least now had an answer and didn't need to worry anymore.
I truly hope he is alive and that you can find an answer. If you haven't tried already, maybe try sending an email and saying that if he wants to breakaway then that's fine, but to please just say if he's still alive/okay. Basically just try to accept him breaking away, while at the same time making him comfortable enough to maybe admit to it.
