So how do spells really work?

The book Silence, and the function of that spell, imply that you need a verbal component for spells to function. Morrowind and the older games had a motion required to cast a spell, implying that the somatic is required. Morrowind even had casting times. But then you have Skyrim, which has casting times and motions for only a few. And you never hear anyone saying anything when casting spells.

So how do you cast spells, really?

From a lore perspective, it's similar to D&D magic (which you sound familiar with), with the added dimension of imagination. In other words, the component of abstract thought. Read this:

The gameplay doesn't really do justice to magic as an "art of the mind", but there's definitely more to it than flailing your hands and chucking fireballs.

In Morrowind, there's a book called "The Four Suitors of Benitah". It describes Fortify Attribute spells in restoration as using raw magicka to create cells in your muscles, your brain, increase pheremones and reflexes, etc.

I'm assuming Magic works by taking pure magicka from Aetherius and manifesting it in the real world in some way.

2920, Rain's Hand is also a neat book that talks about how Psyjics are so proficient with magic by basically abusing the system of how it works.

I love Morrowind because its been a decade and I'm still learning about it.

I once played an illussion-heavy assassin in Skyrim and roleplayed that i'd wisper a few words into my casting hand, like "Your friends will betray you!" or "You're surrounded by draugr!" and "GIANT SPIDERS ARE COMMING FOR YOU!"

Sort of like jedi mind tricks. It was really fun, actually… should try doing it again.
