For those that transitioned after puberty, the physical problems of puberty were already present, and need taking care of anyway. The short term risks for post-puberty are more like roid rage, muscle gain, weight gain, and balding.
Either way, long term risks are fucked up kidneys if incorrect dosages are taken. Diabetes, kidney stones, weight gain (even on a diet), inability to process potassium (so bananas could kill), or complete kidney failure.
Do NOT NOT NOT NOT self medicate! Talk with your doctor. They hear far stranger things than HRT requests. They won't throw you out or yell, although they may try talking you out of it and will most likely request therapy.
Any HRT is permanent after 6 months. Once you have boobs, there is no going back. So most doctors want you to talk it through with a therapist first.
If you absolutely have too have HRT anyway, shop around. There will be therapists or doctors that will short cut the system, just like medical pot.
Official hormones aren't that expensive, even without insurance. A twice yearly blood test and exam is about $400 a year, and hormones are about $80 a month without insurance. Much better than the dried mare piss they sell in Mexico.
For comparison, electrolysis is around $75 - $100 an hour for beard removal, about 200 - 400 hours.
