That "shower head" looks rather like a gas chamber ceiling. Stained paint both on the walls and the floor. Look for scratch marks on the walls if you feel corageous:) although that would be a strange place for a gas chamber.

He was as far in as I went. Bottom of some stairs leading up, but they had filled in the top part. (or caved in) Lots of rocks and rubble completely sealing whatever was behind him.
