When I can't, I name it an average-sounding name that fits the time period.

Depends on the game, usually it's the default name, otherwise it's the name of somebody's pet or somebody I know. If it's something like Pokemon, where I can name a group, I'll pick a theme and stick with it. I keep telling myself I'll be immature and use Faglord one day but I can never find the right game.
If it's naming ships in a fleet I'll either go with real ships or something from the 40k name generator. It depends on what the character limit allows.

For multiplayer games I tend to use the same username I made back when I was in elementary school playing Pokemon. It's fucking embarrassing but I'm really attached to it.
For single-player games I tend to go default. If it's something like Etrian Odyssey I name the characters after other characters I like or I just make them up. In Shin Megami Tensei games I always use the same protag name, which is my Chinese name.

Wit Multiplayer games, getting a unique name gets kinda difficult though. It usually ends up some variation of "Standard", "Default", "Username" or a reference to a chan gneral in-joke about the game if there is one.

At first I used my own name.
When my friend gave me PKMN Mystery dungeon for GBA the name already in use was "Bash". I started to use this name in other games for a while; about a year, but then moved on to a different name.
I don't know why, but when I was younger I tried to avoid the cringeworthy names like blazingshadowmaster24. I picked a more mature name which I still use online today.
I got a bit more creative with my alternate names. I used a different name every year or so, usually with an email to accompany said name.
I used names like Silver, and Damian for rival characters. I didn't like using "poophead" much.
I tried using the names of mythical people subtly, and enjoyed thinking up new names that sounded cool.
I also succeeded in finding a four letter easy to remember name which is never taken, because adding numbers after your prefered name always irritated me (on the internet).
These days I like being called anon.

Now I just name them Bob Sapp. I still find it funny, several games later.
