There's a lot of factors to consider here.
One, why would English speakers buy untranslated games in the first place? How many even know DL site exists?
Two, I emailed a DLsite dev offering to translate a game, he said "ok I'll contact you about it once the details are settled", and hasn't spoken to me in a year.
Three, hentai isn't nearly as popular in the west as it is in Japan - compare the number of hentai games made in English countries to those made in Japan.
Four, the patreons of Akabur and Breeding Seasons should make it clear that the hentai community in the west IS willing to shell out cash… when the hentai is western in both language and design.
Five, Japanese pirate a fuckload too, 2ch is always annoying in that they assume no Japanese person pirates and it all must be immoral gaijins.
In short: it's no surprise that far more Japanese people buy Japanese games made for Japanese people in Japanese, than English people buy Japanese games made for Japanese people in Japanese.
